DEA INFORMATION
As
you are well aware, all practitioners who can legally prescribe
Controlled Substances are required to have a Federal DEA number. This
includes New Hampshire optometrists who are pharmaceutical-certified
(all newly-licensed optometrists are automatically TPA-certified).
Even if you decide not to prescribe any controlled substances at all,
you still need a DEA number. Most pharmacies will not fill any of your
prescriptions unless you have one, and most third-party plans will not
credential you unless you have one.
The simplest way to get a DEA number is to go to the following
website:
http://www.deadiversion.usdoj.gov/drugreg/reg_apps/onlineforms_new.htm
(if
you need a new DEA number) or
https://www.deadiversion.usdoj.gov/webforms/jsp/regapps/common/RenewalAppLogin.jsp
(if
you need to renew your current DEA number)
and follow the instructions you will find there. Optometrists are
considered “Mid-Level Practitioners” and you need to request Schedules
3, 3N and 4. These are the Controlled Substances we are authorized to
prescribe.
There are strict rules and procedures you must follow relative to the
writing of Controlled Substance Rx’s and dispensing Controlled Substance
samples. The Pharmacy Board in Concord or your local pharmacist should
be able to give you that information.
